TAGOLOAN COMMUNITY COLLEGE, located at Baluarte, Tagoloan Misamis Oriental, a duly registered
and recognized HIGHER EDUCATIONAL INSTITUTION, represented by the College President, HON.
ATTY. NADYA B. EMANO-ELIPE, hereinafter referred to as the EDUCATIONAL INSTITUTION-FIRST
PARTY;
and
PUERTO COMMUNITY HOSPITAL, with office address at Zone 6 Puerto Highway, Puerto, Cagayan de
Oro City represented herein by the Billing Department and Hospital Administrator MS. CORAZON A.
CABABAT hereinafter referred to as the PARTNER INDUSTRY-SECOND PARTY.
WITNESSETH, that:
WHEREAS, TAGOLOAN COMMUNITY COLLEGE proposes that its students be allowed to undergo On-
the-Job Training Program at PUERTO COMMUNITY HOSPITAL Billing department located at Puerto,
Cagayan de Oro City which proposal was accepted and approved by the latter, subject to term and
conditions provided for in this Memorandum of Understanding On-the-Job-Training Program;
WHEREAS, TAGOLOAN COMMUNITY COLLEGE and PUERTO COMMUNITY HOSPITAL agrees that
this agreement shall take effect for a period of one (1) year starting from the date of its commencement.
NOW, THEREFORE, for and in consideration of the foregoing premises the TAGOLOAN COMMUNITY
COLLEGE AND FAST SERVICES CORPORATION agrees and stipulates the following:
1. Designate a Program Coordinator who will supervise the On-the-Job Training Program and
coordinate with PUERTO COMMUNITY HOSPITAL on various activities under the On-the-Job
Training Program;
2. Submit to PUERTO COMMUNITY HOSPITAL the resume’ and all other documents required
under the On-the-Job Training Program which shall serve as the basis of the basis screening,
selecting and deploy specific students for the on-the-Job Training Program;
3. Orient students about rules and proper decorum, behavior and work ethics inside the
Company’s premises.
4. Monitor attendance, activities and performance of students during the On-the-Job Training
Program;
5. Monitor compatibility of job circumstances with the course requirements for the student’s
attainment of advanced standing in the program through regular interaction with the student-
training and regular visit to the training site;
6. Together with the students and parents concerned, renounce and waive any claim against the
PARTNER INDUSTRY- SECOND PARTY for injury that the student may sustain or loss that
they suffer, in any form, whether personal and pecuniary arising from the fault or negligence of
the students in the performance of their duties and functions while undergoing the ON-THE-
JOB Training Program.
7. The waiver of the student’s-trainees and their respective parents (as may be necessary) shall
be submitted to the PARTNER INDUSTRY-SECOND PARTY prior to the commencement of
the On-the-Job Training Program. The HIGHER EDUCATION INSTITUTION-FIRST PARTY
agrees and understands that the PARTNER INDUSTRY-SECOND PARTY may refuse to
accept a Student-Trainee without a duly executed waiver;
8. Commit to render excellence in the passion of service to their duty, responsibility and
obligation within 600 hours in its equivalent period of three (3) months and 10 days with eight
(8) hours of duty per day;
9. Training hours shall commence after formal endorsement and ceremonial rites to start 8:00am
to 12:00nn and 1:00pm to 5:00pm from Monday to Friday;
10. Shall not displace a regular worker nor substitute for a worker needed by the Company; and
11. Pull out the student-trainee anytime upon complaint of any violation on the policies of the
industry.
1. Screen, select, and deploy student-trainee to the different offices and operating units.
2. Designate from among its employees an Industry Supervisor who will be assigned the
following responsibilities:
a. Serve as mentor to the student-trainees;
b. Give the student-trainee course-related work assignments and expose them to
learning experience on actual operation and management;
c. Expose the student-trainee to real situation/problem and bottlenecks, thus enabling
them to analyze and apply scientific as well as empirical approach to solution of actual
situation/technical problems; and
d. Arrange a conference with the Dean/Program Director should a student-trainee
problem arises;
3. Commit that the workplace is free from hazardous materials that would lead to untoward
accidents.
4. Terminate anytime the training of the particular students whose contained training/presence
within the industry shall tend to be prejudicial to the interest of the latter and/or any valid
course, but termination can only be done after the joint investigation or evaluation by the
representatives of Tagoloan Community College and the Cooperating firm/Office.
5. Provide the trainee(s) with the necessary protective devices/clothing in hazardous working
area. And shall oblige the student trainee(s) to accomplish their log book of daily performance
to monitor their progress and log book shall form part of their requirements for graduation. Exit
interview and work ethics shall be evaluated as part of their general commendation; lastly,
6. Grant a certificate of completion to the student-trainee at the end of the OJT Training Program
and accomplished evaluation form by the industry supervisor.
This AGREEMENT shall take effect immediately upon signing by all parties concerned and shall remain
enforced and valid. This document consists of three (3) pages to be enforced.
