YOU ARE HEREBY COMMANDED to appear at the offices of PADILLA LAW, 14 NE

1ST AVE, Suite 300, Miami, FL 33132, Phone: (305) 856-1777, Facsimile: (305)856-2411, on or
before (30) days of receipt of this subpoena and have with you at that time and place the following:

1. Any and all 911 calls requesting assistance to address: 4748 Frantz Lane, # 3, Orlando
Florida 32792 from January 2nd, 2017, through to March 1st, 2020.

These items will be inspected and may be copied at that time. You will not be required to
surrender the original items. You may obey this subpoena by providing “legible” copies of the
items to be produced to the attorney whose name appears on this subpoena on or before the
scheduled date of production. You may condition the preparation of such copies on the payment of
the reasonable cost of preparation; however you must first contact the attorney for approval if the
cost will exceed $50.00. You may mail or deliver the copies to the attorney whose name
appears on this subpoena, thereby eliminating the need for your appearance at the time and
place scheduled production. You have the right to object the production pursuant to this subpoena
at any time before production by giving written notice to the attorney whose name appears in this
subpoena.

If you fail to:

1) appear as specified; or
2) furnish the records instead of appearing as provided above; or
3) object to this subpoena,
you may be in contempt of court. You are subpoenaed to appear by the following attorneys
and unless excused from this subpoena by these attorneys of the Court, you shall respond to this
subpoena as directed.
 I HEREBY CERTIFY that I gave notice to every other party to this action of my intent
to serve a subpoena upon a person is not a party to this action, directing that person to produce
documents or things without deposition. I also certify that no objection has been received by the
undersigned within ten (10) days of service of notice, if service was by hand delivery or appropriate
facsimile transmission, and within fifteen (15) days if service was by mail.
