INSTRUCTIONS FOR UPLOADING DOCUMENTS

UPLOAD You must upload two documents: your identification

document and the PIP implant cards.
DOCUMENTS
Check the specifications that must be met by each document before uploading it.

Your PIP cards: Your identification document:

The file to be uploaded must be in PDF format.
If you have more than one card, include all of
them in one single file.
The cards must be labeled with your name.
The fads must show the batch number and
serial number of the implants.
The document must be legible to make sure
they are accepted as evidence in French courts.
The file you upload must be in PDF format.
If your document has two sides, upload both
sides in one same page.
The document must be legible to make sure
that your data matches the one registered in
the platform

After uploading your documents, our representatives must check the contents
CHECK and legibility. The status of your documents will appear on the platform as:

DOCUMENT STATUS No document

Revision pending (our team must check the uploaded document)

Approved

Change pending (you must modify the uploaded document)

If any adjustments to the documents are necessary, you will receive an email with the instructions of the change.
You must sign in with your username to the platform and upload the documents again.

SIGNING THE POWER Once the two documents have been approved, you will receive an email with the

Only women who have completed the entire registration process and who have received

was correctly received, may be part of the judicial process through Lozano Blanco &
Asociados.
