Padilla, to represent me in submitting and handling all necessary documents and requirements

for my application for the PL/DL Academic Scholarship for 1st Semester A.Y 2023-2024.

I am unable to personally attend to this matter due to the distance of the campus from

my hometown as well as the expenses, and I trust Ms. Padilla to act on my behalf in passing

my requirements. She is aware of the specific documents required and is fully informed about

the application process.

I kindly request that your office accept Ms. Padilla’s submissions as if they were
