AFFIDAVIT

I, Niraj Kumar son of Suresh Prasad Singh Age:- 26 years, Occupation:- Student,

Residing at:- Dange Chowk, Wakad, Pune 411 033, permanent residence of K/

53, P.C. Colony, Hanuman Nagar, Kankar Baug, Patna 800 020, state on solemn

affirmation that:

1. I am student having Roll No. MBA I B 26, of Indira Institute of

Management, Near Bombay Pune Highway, Wakad, Pune and I am

residing at the above mentioned place in Pune.

2. I owned a mobile hand set Nokia Model 2100, purchased by me from

Digital World, Pune having handset No. I 351529002864001. My mobile

cell service provider was Idea cellular having No. 98228 88954.

3. On 07/07/2004, I have lost the said hand set from my possession when I

was at ICICI Bank, Apt Road Branch, Pune at about 2 pm.

4. I desire to file a police complaint of the loss of my hand set and the SIM

Card, so as to prevent any misuse of the same and therefore I am

executing this affidavit.

The contents of para 1 to 4 above are true and correct to the best of my

knowledge, information and belief and in the testimony of the same I, Niraj

Kumar, have set my hands at Pune on the day and date mentioned hereinafter.
