Instructions:
1. JSA is not an individual job rather it should be done by selected team members to effectively identify hazard and provide control measures accordingly.
2. It shall be conducted prior issuing work permit.
3. Custodian to appoint competent person as issuer who shall conduct JSA along with Initiator, area supervisor and other competent.
4. JSA should be made as per actual steps involved in the activity.
5. There shall be no name, only relevant stakeholders (issuer, initiator, custodian, isolator, gas tester, co-permittee) to be mentioned in responsibility column.

Requests ensure wellbeing check of staff before initiating the activity.
