Dear Dr. Subita ,

EXPLANATION LETTER : INCIDENT OF NOT TRACING ADMISSION BLOOD

Regarding to the above title, it has been made aware that I as a houseman should trace and inform
oncall MO regarding any abnormal blood result.

On the day of the incident I was working short shift. A shift from 7am-5pm. Patient was
admitted to the ward at 330pm on 5th October 2020 ( according to the nurse report ). Subsequently
patient was brought to the bed and attended by HO, MO and Specialist at 4pm. Clerking and
documentation took around 20 minutes to finish and we started to prepare for blood taking at 430
pm after investigation was ordered. Blood was taken at 445pm and later was sent at 505pm by me
after my shift ended.

I believe that I’ve done my part and as a houseman on that day working on short shift from
7am – 5pm. Hence in my humble opinion I shouldn’t be put responsible. I am sorry for not being able
to trace the result as my shift ended and its going to take 2-3 hours before the result is out.

Non the less, I will do better in the future and will not make such mistake. Medical
Department has been a great experience for me. I would like to apologise for such inconvenience.
