Incident Report

December 3, 2022

On the day dated, December 3, 2022 I, Daniel Angelo Tanyag , and Decebert Ortiz scheduled for the
Night shift. After we received the turn over from the Mid shift I assigned the Enercon task to Decebert
Ortiz and I asked if he already know the Night shift routine because he was being assigned to Graveyard
for many weeks already. After checking the All day dining area I call Decebert to go the mezzanine
electrical room because I will oversee the circuit breakers to be turn off and the Following happen;

Me: Sir alam mo yong papatayin na may mga blue mark diba, sige sir ioff mo isa isa tingnan ko.

And then Bert open the one panel and did not off anything and I said

Me: Sir dpat inaalam mo yan. Lagi ka ng GY e.

And he got furious about what I said and say

Bert: Sir hindi naman ako electrical e. pti may kasama ako kpag umiikot kami dito.

Me: Pano yan sir 2 lang tayo ngayon kaya nga po tinuturo na sayo e

Bert: Hindi ganan magturo sir minamahina mo ako e. Kung ganan ka mag turo sir Uuwian kita Putangina!

Me: Sige umuwi kana. And then Bert walk out the Electrical room

So I immediately called the Midshift plumber, John Abarca to go on straight duty knowing that Bert will
go home already. After I finished switching off all the lights and AHU to be turned off I go back to B1
base to meet mid shift Plumber John Abarca and Bert is still there, a little bit calmer and I ask if he like to
work still and said yes.
