10 DRILLING/OPERATIONS REPORTING (WOS 10)

10.1 General
For all Lasmo well operations there must be a daily written record of each day’s activities,
transmitted daily to the Lasmo Well Operations Manager. The Lasmo Well Operations
Manager shall ensure that reporting complies with Lasmo and statutory requirements.

10.1.1 The basis of reporting for rig-operated activities shall be the official IADC API Tour Report.
Before signing the report, the Lasmo Well Operations Supervisor should ensure the
following:

(a) All drilling details and parameters stated are accurate and true.

(b) Time breakdowns are accurate and the correct time code applied.

(c) Bottom hole assembly (BHA) weights and weight below the jars are recorded.

(d) All pit drills, blowout preventer (BOP) drills, fire drills and all other drills are recorded.

(e) Safety meetings are recorded.

(f) Crew details are correct.

(g) Hours on jars, shock-subs, motors etc are recorded.

(h) The report has been signed by the Toolpusher and both Drillers.

(i) Daily BOP functions are recorded.

10.1.2 The Lasmo Well Operations Supervisor shall compile and transmit a Lasmo Daily
Drilling/Operations Report to the operations base in a format and at a time approved by the
Well Operations Manager.

10.1.3 The following minimum reports shall be completed, transmitted and filed:

(1) API mud report.

(2) BOP and other pressure tests.

(3) Formation integrity tests (FITs) and leak-off tests (LOTs).

(4) Daily personnel on board (POB) or similar.

(5) Well handover certificates.

(6) Casing and cementing reports.

(7) Incident and Accident reports.

(8) Materials manifests.

(9) Start of well/end of well, inventories.

(10) Completion diagrams.

(11) Suspension/abandonment diagrams.

(12) Site clearance reports.

(13) Safety meeting minutes.
