AFFIDAVIT OF LOSS

I, ELENITA BACORRO LABAO, of legal age, Filipino citizen,

single, with postal and residence address at No. B 1 L 16
Patricia South Villa Brgy. Anabu 2-F, Imus City, Cavite,
Philippines, after having been sworn to in accordance with law
hereby depose and state:

1. That, I have a clothing store in Imus Public Market in

Brgy. Bayan Luma 2, Imus, 4103 Cavite, when a
customer paid for his purchase while I was making
change, he saw that I had taken change money from my
black back pack for a while I put it aside and I was busy
attending the needs of the said customer. I did not
notice that he had already taken the said black pack
bag of mine;

2. That, the abovesaid black back bag contains personal

belongings, the following such as:

3. That, the aforesaid black back pack contains

Identification (ID) cards and others, the following such
as:

 TIN Card
 Postal ID Card
 Philippine Passport with reference number
P2786076B
 Cash value of P 20, 000.00

4. As such, I am executing this Affidavit of Loss to attest to

the truth of the foregoing facts and for whatever legal
purposes it may serve.
