Dear Dr. Mohamed Farag,

The LAR Masterclass - Acibadem University - Istanbul - Turkey - October 17-18,2022 will be held in Acibadem University in Istanbul on
October 17, 2022). This event will provide continuous medical education with regard to new products and therapies in the field of .
Medtronic considers the support of such specialized events to be part of its contribution towards optimizing patient health care. A copy
of the program of this event has been attached for your convenience.

We hope that the information gained during this event and the possibility to share your experience and open a dialogue with other
specialists will broaden your knowledge with regard to new products and therapies in your field of expertise. We also hope that this
experience will help you to develop educational programs at your medical institution and to share acquired knowledge with your
colleagues.

In accordance with Medtronic’s expense reimbursement policies, Medtronic shall organize and cover the following costs for your
participation in the above event:

• Meeting registration
• Ground transportation (Medtronic organized transfers)
• Economy class airfare
• Train Ticket
• Lodging: 3 nights
• Breakfasts, dinners and lunches organized and directly paid for by Medtronic
• VISA costs

Please note that Medtronic shall not pay for any other costs than the ones indicated above. In addition, we confirm that an extension
of your stay cannot be organized or reimbursed by Medtronic.

With your attendance at the above-mentioned event, you acknowledge that you are responsible in taking all necessary steps to protect
yourself and to follow any applicable laws and government measures pertaining to the COVID19. You also confirm your agreement
with this arrangement and that you have authority to receive our support for this event and that you have followed the rules and
policies of any institution or professional association with which you are affiliated. You furthermore confirm that you have duly informed
your hospital administration, supervisor or other locally designated competent authority about Medtronic’s invitation/support, and that
you are authorized by your hospital administration, supervisor or other locally designated competent authority to attend the above-
mentioned event. The attached notification letter may be used for such purpose.

Personal Data of Participant or of third parties. Please find attached a privacy notice detailing how Medtronic processes Participant
personal data.

Participant will not provide any personal data of third parties, including personal data of patients to Medtronic, except where agreed
between parties in order to allow parties to demonstrate compliance with applicable laws, regulations and applicable government or
industry guidelines with regard to interactions between medical device industry and healthcare professionals.

We would like to state that this Agreement does not create any obligation or expectation for you or your medical institution to use,
promote or purchase Medtronic products.
