PLAYER REGISTRATION FORM

To be completed by ALL members (both player and non-player) and returned with payment of annual
membership fee. A parent/guardian must complete form for players under the age of 16.
One form is required per member. Please complete ALL sections as appropriate.

As a player / parent, it is important that you disclose to your Team Manager any information relating to safety and
welfare of you / your child. This information may relate to any of the following:
- health needs such as asthma, diabetes, epilepsy, etc
- current medications which you / your child may be taking
- communication needs (speech impairment, hearing difficulties, etc)

I give consent, for the purposes of the Data Protection Acts, to the Club holding and processing the data on this form
for the purposes of the Club including using the data to communicate with members in relation to matter relating to
the running of the Club and Club activities.

PARENTAL CONSENT (to be completed by parents of players under 16 years of age)

I give consent for my child to participate in CP Football Development Academy's activities and events and accept that
it is my responsibility to inform the Team Manager directly of any particular requirements for my child. I hereby give
consent for photographs and images of my child to be taken and use solely for team or club purposes including
publication on the Club's website / facebook page.

CODE FOR PLAYERS

As a registered player of the Club, I agree to the following:


1) To play by the rules.
2) To never argue with an official.
3) Control my temper. I accept that verbal abuse of officials and other players, deliberately distracting
or provoking an opponent are not acceptable or permitted behaviours in the Club.
4) To be a good sport, applaud good play made by either my team or the opposition.
5) To treat all players and team mates as I would like to be treated.
6) I will co-operate with my coach, team mates and the officials of the club.
7) To respect the rights, dignity and well-being of all participants regardless of their gender, cultural
background, ability, or religion.
8) If I have concerns to talk to my coach or to a member of the committee.

CODE FOR PARENTS (not required for senior team members)

As a parent or guardian of a registered player of the Club, I agree to the following:


1) To remember that children participate in sport for their enjoyment, not mine.
2) To encourage children to participate, not force them.
3) To focus on the child’s participation, and not on winning and losing.
4) To encourage children to always play according to the rules and to settle disagreements without
resorting to hostility or violence.
5) To never ridicule or shout at a child for making a mistake or losing a competition.
6) To remember that children are led by example.
7) To support all efforts to remove verbal and physical abuse from sporting activities.
8) To respect the officials’ decisions and teach children to do likewise.
9) To show appreciation for volunteer coaches and officials.
10) To respect the rights, dignity and worth of every young person regardless of their gender, cultural
background, ability, or religion.
11) If I have any issues to do with my child, I will discuss with their manager or a committee member.
12) Not to discuss other players in public.
13) To support my child’s manager, and be proud of my child’s progress at all times.
14) To support the club in the long term football development of my child.
15) To ensure my child is dropped to and collected from club training and activities at the agreed time.
