AFFIDAVIT OF VOLUNTARY
RELINQUISHMENT OF PARENTAL RIGHTS

_____________________, after having duly sworn to in accordance with
law, hereby depose and states: That –

1. I was married to ______________________ on AUGUST 18, 2004;

2. Our marriage was blessed with two (2) minor children namely:

a. ________________________ who was born on NOVEMBER

11, 2004; and
b. ______________________ who was born on DECEMBER 6,
2007.

3. I am the biological and legal father of the above-mentioned minor

children;

4. My marriage to __________________________ was eventually

judicially declared null and void on February 15, 2018;

petition and sole parental authority and custody of our minor children;

For the welfare and best interests of our children, I am giving my full consent
and authority for the custody, support and care of our minor children, in

giving and waiving all my rights and interest to oppose any and all legal
matters that my former wife will do regarding the custody of our
children, and for whatever legal purposes it may serve.
