AFFIDAVIT OF ONE AND THE SAME PERSON

I Elizabeth D. De Castro, Of legal age, Filipino citizen, Married and a resident of Calao,
Dumangas, Iloilo After being sworn in accordance with law, hereby depose and say that:

1. I am the child of FEDERICO DEFIÑO AND CARINA D. DEFIÑO;

2. I have used the name Elizabeth Dechavez Defiño, in all of my records and dealings such as
government IDs, school records, baptismal certificate and other of similar nature;
3. The name registered in the birth certificate issued by the PSA is ElizabethDefiño.
4. I got married on December 28, 2018;
5. After marriage I change my name to Elizabeth D. Defiño-De Castro;
6. This affidavit was executed to attest to the truth in the forgoing and for the purpose of
correction of entry in the Civil Registrar in my favor and for whatever legal purpose it may
serve.
