Foot Injury While Using Slips

July 2022
The Alerts are provided to all Drilling and Workover Operations Foremen and Liaisonmen so that we all learn from the
experiences of others. The knowledge of this incident could prevent similar ones from happening in the future.
Saudi Aramco policy requires the information in this Alert to be communicated to all crew members and documented on
morning reports.

What happened?
Operation was tripping in hole with 4” drill pipe. The floorman (IP) was preparing
to set the slips. As he pushed the slips into the bushings, his right foot slipped and
came in contact between slips segments and the rotary table bushing trapping his
right foot’s big toe.

Consequences:
• Personnel Injury.
• Operations interruption

Why did it happen?


• IP was working alone on the rotary table handling the slips (Wrong Practice).
• There was no Risk Assessment done to cover the hazard of foot placement
and how many crew members should be working on the slips as required.
• Stop Work Authority was not exercised to stop the wrong practice.

What should be done to prevent recurrence?


• Refresher trainings shall be conducted to all crew members on line of fire
incidents and on their job functions (example: Procedure of handling slips).
• Update the Risk Assessment to include hazards and control measures for foot
placement & required number for drill crew that need to be involved while
using slips.
• Ensure SWA is reinforced among all crew members and accountability
communicated.

Area where improvement is needed:

Lifesaving Rule No.1 Work Permit.

Workers must:
Stop work if permit conditions change, and
notify the supervisor.
Supervisors must:
Confirm that permit conditions are met and all
required resources are at the worksite before
work starts.

Lifesaving Rule No.8 Danger Zones.

Workers must:
Always Maintain Awareness of the
surrounding.
Supervisors must:
Confirm that all hazards are identified and
controlled.
