The patient has a history of schizophrenia with five prior admissions. Recently, he has been non-compliant with his medication except chlorpromazine. He has been irritable towards his mother and had instances of knocking on her head. He also threatened to stab his father, siblings, and cousins while holding a knife. Upon admission, his blood tests showed increased fasting blood sugar, cholesterol, and LDL but decreased sodium. He was started on medications to address these issues and his sodium levels normalized by November 23rd. He was diagnosed with schizophrenia in October 2022 and was last admitted in 2019. In the month prior to this admission, he became more aggressive, pushing his mother in anger while experiencing auditory hallucinations and delusions

Schizophrenia, with five prior admissions. Last October to November 2022, he has no OPD follow-up done. The patient is non-compliant to medication except to Chlorpromazine and other medications that make him feels like a robot. The patient would get irritable to his mother saying she is getting money from Trump, Biden, Pacquaio, and Villar. The patient has instances of knocking on the mothers head few days prior the admission. The patient is holding a knife saying he will stab his father, siblings and cousins.
On November 18, 2023, it was noted that his Fasting Blood
Sugar and his Cholesterol level has increased so his LDL increased but his sodium decreased. The patient started taking metformin 500mg 1 tab PO BID, Gliclazide 80mg 1 tab OD, Fenofibrate 160mg 1 tab OD and oral rehydration salt in 1 sachet in a h glass of water. Then on the Nov. 20, 2023 fenofibrate shifted to Rosuvastatin, then his sodium has been normalized last Nov 23, 2023.
The patient is diagnosed case of Schizophrenia last Oct. 17
2022. The last admission was 201 9, since the discharge of the patient he has fair compliance with his medication 2 months prior to admission. The patient was observed to have labile mood, easily angered, received an energy with auditory hallucination and delusion of working in the USA.
1 month prior to admission he became more aggressive and
would push his mother with anger. He was aggravative and was brought for consult, ideally, he is for admission but there was no vacancy. The patient behavior worsened and was brought for admission when his mother learned that there's may be vacancy.
The patient has 4 previous admission last July 2019-
January 22 yet has no specific information in the chart
