VOLUNTARILY allow my child ____________________________________ (Name of Student) to
participate in the training for the upcoming Intramurals 2023. The said date of training is
on January 21, 2023 from 7:00 to 4:00. I fully understood that / I my child may be
exposed or infected with COVID 19 by attending and participating in the said training
and such exposure or infection may result in personal injury, illness, permanent disability.
I hereby waive, release and discharge HOLY CROSS COLLEGE from any damage, injury,
illness, loss or death that may occur to my child by participating in the said training.
