AFFIDAVIT OF NON-EMPLOYMENT

I, HARLENE ABUGAN AGUSTIN, Filipino, of legal age, married, and a resident of

Brgy. Oscariz, Ramon, Isabela, after having duly sworn in accordance with law, do hereby
depose and state that:

1. I intend to file for correction of entry in my Certificate of Live Birth at the Municipal
Civil Registrar’s Office in Alfonso Lista, Ifugao, particularly the correction of entry
no. 2 (“Sex”) from Male to Female;

2. That among the requirements for the petition of correction is an Affidavit of Non-
Employment;

3. To this date, I am not employed in any capacity at any corporation, company,

individual person or with the government;

4. I am executing this Affidavit of Non-Employment to attest to the truth of the

foregoing and to support my application for Correction of Clerical Error for R.A.
10172 in my Certificate of Live Birth before the Office of the Municipal Local Civil
Registrar of Alfonso Lista, Ifugao and for whatever legal purpose this may serve.
