    Mr. Ijaz Ahmad caused an accident on November 11, 2023 at 6:55 PM near the store at the Thumrait site. While taking a turn recklessly without observing vehicle margins, Mr. Ahmad hit a parked vehicle driven by Mr. Adnan Mohammad with his vehicle. The accident caused damage to the coolant tank costing 110 OMR and a broken side mirror costing 6.5 OMR, totaling 116.5 OMR in damages. As a result, Mr. Ahmad was fined through a salary deduction for the damages and warned that any future incidents would result in a suspension from duty.

    Incident Location: Near store, Thumrait site.
    Vehicles Involved:
     Vehicle 1 : HINO-07 Reg: 1120 WB Driver: Mr.ADNAN MOHAMMAD(MT255).
     Vehicle 1 : HINO-19 Reg: 7756 HM Driver: Mr.IJAZ AHMAD(MT270).

    Description: As per investigation and Pattern of damages occurred , HINO-07 DRIVER:


    Mr.ADNAN MOHAMMAD parked vehicle near store to sign out his attendance of DAY-SHIFT
    END. HINO-19 DRIVER: Mr.IJAZ AHMAD Caused accident while taking turn.

    Conclusion:
    HINO-19 DRIVER: Mr.IJAZ AHMAD caused the accident with negligence while recklessly taking
    a turn with out observing the vehicle margins.
    Damage caused:
    1. Coolant tank is Damaged Cost:110 OMR
    2. Side Mirror is Broken Cost: 06.5 OMR
    Total : 116.5 OMR
    Action:
     HINO-19 DRIVER: Mr.IJAZ AHMAD is FINED (with Salary deduction) of damaged costs .
     He is Warned to work cautiously “if again repeated will suspended from duty “.
