HSE ALERT
“ROAD TRAFFIC ACCIDENT”

What happened:
The CWC site team was preparing the new area and
arranging concrete barriers for pipe line excavation
works at Alkhwarzmi road, when at 12:17 PM a
speeding car hit the concrete barrier and flag man
stationed inside the concrete barrier leading to
injury. Following accident, ambulance was called
immediately and the victim was transferred to
hospital at 12:30 PM.

Root Causes:
1. All the attention of the car driver was focused on the mobile phone.
2. The flagman signaled the car driver not approach the concrete barricade but didn’t
lock toward the flagman.
3. The car driver didn’t care about the road speed because he was busy on mobile.
4. Due to overspeed the car driver hit the flagman.

Learnings:
1. The use of mobile phones on the project while driving on the road site is strictly
prohibited for all managers, engineers and inspectors. This will be achieved by
conducting frequent awareness programs for the entire NWC contractors and ILF
consultant community. Provide status report to NWC in case of any road accident
while using mobile phone.
2. Do not speed and follow the speed limit.
3. In case of any accident the victim shall be taken directly to nearest Govt. Hospital
rather than to the private hospitals.
4. Specific Road Safety risk assessment shall be prepared with an emphasis to manage
road safety taking into account of learning points ferreted out of this accident.
5. Regularly check the preventive maintenance records of all site vehicles.
6. Ensure all the drivers are trained for Defensive Driving techniques.
7. All the focus shall be on road while driving.
Each Contractor and Consultant site engineer and inspector are responsible to
take appropriate action to implement the above learnings and prepare the
project specific driving Safety requirement covering all above learnings.
Formal confirmation report to be send ILF HSE Manager maximum within 03 days
upon receipt of this Alert.
