I, MARLON BRAZA ARPIA, of legal age, Filipino, married, with

postal and residence address at No. B 25 L 27 Green Mark Subd.,
Brgy. Malagasang II-C, Imus City, Cavite, Philippines, after having
been duly sworn to in accordance with law, hereby depose and state
that:

1. I am the registered owner of a motor vehicle, further described

3. That, this affidavit is executed to change the Body Type of

abovesaid motor vehicle from Motorcycle with Sidecar to
Single without Side car. I am also executing this affidavit to
attest to the truth of the foregoing facts and circumstances
and in support of my request to update the above-stated motor
vehicle record in the Land Transportation Office.
