    MINORS v. BRANDI THOMAS, In the 127th Judicial District Court of Harris County, Texas.

    Dear Counselors:

    Please be advised that I have been appointed Guardian Ad Litem for the minors by the Court
    in the above referenced matter. Find attached the Guardian Ad Litem’s Original Answer for the
    minors.

    I would request that the following be provided for my review: minors’ DOB; minors’ social
    security #; police report; photographs; insurance declarations page (1st and 3rd party); documents
    evidencing any PIP benefits/claims; medical bills; medical records; discovery; settlement amount;
    settlement disbursement sheet; release; judgment; and any other settlement documents on the
    aforementioned. If there were any other settlements related to this incident, please provide settlement
    disbursements for each settling individual. If this is a policy limits settlement, please provide
    documents evidencing asset checks/searches on all defendants. Finally, please provide the address,
    phone number, and email for the next friend.

    Feel free to e-mail me any of the documents requested.
