Upon admission

from FCP.
2. Secure copy of Identification
(ID) from FCP or family members.
3. Medical department will
administer interview to FCP (FCP
interview guide) and program is required
to sit in during interview to acquire
significant information about the
prospect.
4. Turn over to admin the inquiry

Program dept. must monitor the following:
 Admin must secure admission contract.
 Admin will also post that all documents
upon admission has been secured.
 Program must also provide the medical
department all pertinent waivers and
inquiry information

processes that the Resident will
undergo such as medical
requirements and labs, usual
activity and routine in the main
house etc.
7. Program will update COC and
secure FCP chat group which
includes medical dept. for proper
dissemination of information.
8. Program must conduct

emotional interview and let
Resident-client sign consents to
participate and secure admission
form.
9. Endorsed all consents, waivers,
and necessary forms to CLM dept.
CLM

1. Initial assessment / Brief MSE

2. CLM will coordinate to Program
concerning observation and/or
results of the assessment
3. CLM will provide medical dept.
the initial assessment results for
reference during Psychiatric
consultations.
4. FCP interview output will also be

provided upon Initial consultation.
PROGRAM

specific position

to CLM dept. and observe
resident’s progress upon
admission and within 2 to 3
months period (case to case basis) to
prepare resident-client for
psychological testing.

conceptualize the overall condition
of the Resident-client and provide
efficient treatment plan (program
matrix formulation).

explicate program matrix to FCP.
