CONTRACT OF SERVICE

KNOW ALL MEN BY THESE PRESENTS:

This SERVICE CONTRACT entered this ____ dau of ____________ in the Municipality of Del
Gallego Camarines Sur, Philippines, by and between:

The BARANGAY LOCAL GOVERNMENT UNIT OF POBLACION ZONE I, DEL GALLEGO, CAMARINES
SUR, a Barangay local government unit created under Executive order
_______________________________________ and with office address at Barangay Poblacion Zone I Del
Gallego, Camarines Sur, represented in this contract by it Punong Barangay- HON. OFELIA D. BUENDIA,
hereinafter referred to as the office.

And

The persons whose name and address indicated hereunder, hereafter referred to as the service
contractor-- ______________.

WITNESSETH:

WHEREAS, The BARANGAY LOCAL GOVERNMENT OF POBLCAION ZONE I, DEL GALLEGO, CAMARINES
SUR, needs extra manpower to augment the manpower to conduct a demolishing team in these
barangay;

WHEREAS, the Service Contractor offer their services to perform the task of contractual
________________;

WHEREAS, the Barangay Local Government Unit finds hiring of Contractual Demolishing Team
most advantageous to the Barangay Local Government Unit.

NOW THEREFORE, and in consideration of the foregoing premises and of the terms and
conditions hereinafter provided, it is hereby agreed that:

1. The Barangay Local Government Unit shall hire the Service Contractors with the daily rate of
FOUR HUNDRED FIFTY PESOS (PHP 500.00) effective July 12,2023 to July 25, 2023. They shall be
paid upon performance of contracted work and standard output per day;
2. The Barangay Local Government Unit shall provide a local person and supervisors to supervise
and monitor the activities and accomplishments of the Service Contractor;
3. The Service Contractor shall work Mondays through Saturdays on an eight hour per day work
schedule.
4. Termination:
A. This Service Contract may be terminated at any time during its effectivity in the event of
unsatisfactory performance of work or when the person hired under the service contract
deliberately violate the provision of the contract. In addition, immediate dismissal may
be resorted to in the event that the person hired under the service contract is prima
facie found to be, directly or indirectly, involving in the following activities
