GENERAL SITE CONDITION

1 Posting Safety and Other job site warning posters

2 First Aid Kits, Stretcher and Blanket available at Site Office

3 Emergency telephone numbers (Police Dept., Fire Dept., Doctor, Hospital, and
Ambulance) posted in the proper places

4 General cleanliness of working area / construction site

5 The site safety committee up dated

6 It the site fence or hording board constructed at least 12 meter height at the
boundary of the site to prevent unauthorized person entering the site

PERSONAL PROTECTIVE EQUIPMENTS

22 Employees are provided with proper protective equipment’s (helmet, safety


shoes, glasses, masks, work gloves, safety belts, etc.). And all of them are used
appropriately

23 Protective clothing provided and maintained adequately clean and in good repair

24 Employees are provided with proper eye and face protection equipment when
machine or operations present potential eye or face injury from physical, chemical
or radiation agents

25 Life lines, safety belts and lanyards are used only for employees safe guarding,
and all are matched testing requirements

26 Safety belts lanyards is a minimum of ½ -inch nylon or equivalent with maximum


length to provide for a fall of no greater than 2 meters

27 Safety nets are installed when work places are more than 25 ft. above the ground,
or other surface where the use of ladders, scaffolds, temporary floors etc. is
impractical

28 Safety equipment worn correctly and is constantly monitored by supervisors

HAND TOOL MANUAL AND POWERED

31 Workers who use hand tools are familiar with the tools and know how to repair
and maintained them

32 Belts, gears, shafts, chains or other reciprocating, rotating or moving parts of


equipment are properly guarded if the parts are exposed to contact by employees

33 Impact tools (drift pins, wedges and chisels) are kept free of mushroomed head

34 The wooden handles of tools are kept free of splinters or cracks and are kept tight
in fixed to tools e.g. hammer / axes

35 Electrical equipment using water (concrete cutter, concrete boring machine


masonry cutter etc.) are grounded to prevent worker being electrocuted when in
use

36 Air tools secured on air hose coupling in order not to snap

37 Hydraulic tools checked, no ruptured or broken hoses etc.

LADDER

38 There is no obstruction around ladder (both top and bottom)

39 Top and bottom of ladder are fixed firmly

40 Ladder used in good condition, no rungs missing or broken

41 Landing surface is not muddy or wet but clean and dry (muddy condition causes

Slipping and falling hazards due to muddy feet)

42 Ladder are not being used for the job which required the scaffold

43 When ladder cannot be secured at the top for technical reasons, ensure that they
are secured near the bottom or footed

44 Ensure metal ladder are not being used near overhead power lines

45 Inspect ladders regularly for sign of damage or corrosion

SCAFFOLD

46 Scaffold walkways free of debris and grease

47 Working platform is enough wide to work safely with proper guard-rails and toe
boards

48 No scaffold including accessories such as braces, brackets, trusses, screw legs,

49 All planking of platform is overlapped (minimum 12 inches) or secured from


movement

50 Ensure that no parts of the scaffold are removed or loose after it is erected

51 Make the upright of the scaffold vertical and securely braced to prevent swaying
or displacement

53 Arrange the boards so as to avoid tipping or tripping risks

FIRE PROTECTION

54 Provide sufficient number of fire extinguisher at all critical points and easily
accessible when need. Fire orders at each point

55 The fire extinguisher are periodically serviceability

56 A alarm system (bell system, siren. etc.) is established by the employer

57 “NO SMOKING” OR “OPEN FLAME” signs are conspicuously posted at or in the


vicinity of operations which constitute a fire hazards. (E.g. paint storage
workshop)

58 Keep the flammable liquid or LPG cylinders in a safe place when not in use

ELECTRICAL

60 Ensure that distribution cables are not causing a hazard at opening, passages,
ladders, stairs or lying on ground subject to damage and wetness

61 Not hanging directly from nails, etc. which may damage insulation and well
protected from sharp objects.

62 Suspended cable not carrying any weight and should be supported by rods or
poles

63 Ensure that all cables are visible. If necessary attach (yellow and black or red any
white) plastic bending strips

64 Ensure that there is no adequate supply of extension cable for site use

65 Remove from the site all unproved junctions i.e. “spiders”

66 Plug covers etc., are not damaged and splash proof ones used

67 Check that the correct plugs are fitted-(especially by the sub-contractor)

68 Check that the connection have been made color coding should be distinctive

69 Check that cable grips are used and that the earth cable s fitted with same slack
So that it is the last to be pulled out

70 Ensure that improvised junction (nails, matches etc.) are removed

71 Ensure that the correct type and rating of fuses are fitted and checked by
electrician

72 Ensure that improvised fuses (nails, silver paper etc) are removed and ideally
have fool proof earth leakage breaker fitted

73 Ensure that any special “make up” leads / extension are made by electrician
(competent person) only

MOBILE CRANE /TOWER CRANE

74 Make sure that every crane is thoroughly examined at regular intervals by


competent person according to government (FMD) requirement and manufacturer
recommendation. A test certificate obtained for each crane. PMA number

75 Maintain every crane regular for wear and tear. Record of servicing maintained at

76 Ensure that every crane is inspected weekly by competent person and that a
record kept

77 Ensure that every crane operator and signalers are trained in the of use of hand
signal and ensure that their illustrations are displayed on the job site / cabin

78 Ensure that every crane of more than 1 ton capacity has efficient automatic safe
load indicator which is inspected weekly and record kept

79 Provide a safe mean of access to all parts of the crane to which it is necessary to
go for operation and maintenance

80 Train the operator and ensure that the age of the operator is over 18

81 Ensure the operator is competent operator and registered with DOSH

82 Ensure that the crane operator is provided with safety equipment i.e. helmet and
safety belt

83 Ensure that the operator and maintenance personnel are fit before they are allow
to climb to the cabin or jib

84 Base of crane clear from obstruction, is clean and no stagnant water

CONCRETE CONSTRUCTION

85 Concrete workers protect themselves with boots, heavy gloves, long sleeved shirt,
safety helmet

86 Each iron worker (reinforced concrete) is not too close together and the periphery
of the slabs are properly guardrails

87 Stripped forms and shoring are removed and stockpiled promptly

88 Protruding nails, wire ties and other unnecessary accessories are immediately
pulled, cut or taken

89 Walkways are kept clean of spilled concrete

90 Check that the design and supports of form work and shoring are adequate to
support all intended load

91 Ensure that the form work is inspected by competent person before pouring of
concrete begin

92 Wear gloves and eyes protection when stripping form work

93 Immediately remove or band over nails from stripped form work

POWER TOOLS

94 Check for SIRIM or maker marks

95 Check that the cable is protected from excessive flexing by the heavy rubber
sleeve where the cable enters the tool. Cable clamp is secure

96 Check that there are no crack or pieces missing from the machine. All screws are
in place and secure

97 Check that the chuck is in good condition and that the correct key is attached

98 Check that the name plate is secure .Detail available on the machine plate such
as make, voltage frequency, speed etc.
