AFFIDAVIT

I, RAYMOND ANTHONY BEGUÑA, of legal age, Filipino, married, and a resident of P13-B,
Prince Kim Subd., Barangay San Vicente, Butuan City, under oath, depose and state:

1. That I was born on November 9, 1979 at G.P., Caloocan City to BERNADETTE Y. BEGUÑA;

2. That since I was young, I have always been known and called “RAYMOND ANTHONY” and it
is the same first name that I have been using in all my documents – a proof that I was born
MALE;

3. However, it appears from the records of the Office of the Civil Registrar of Butuan City that I
was registered therein without a “SEX”

4. That it is my desire to be registered with the said Office of the Civil Registrar of Butuan City with
MALE as a “SEX”.

5. That the failure to state my SEX was purely the inadvertence of my parent who failed to indicate
so at the time of the registration of the facts of my birth with the said office;

6. That I am executing this affidavit to attest to the veracity of the foregoing for whatever legal
purpose it may serve.
