2024 and is married to _________________;

3. That the date of birth appearing in the said person’s Certificate of Death
issued by the Office of the Civil Registrar General is “August 18, 1941”;

4. That the date of birth appearing in the GSIS Database was entered as
“August 18, 1942”;

5. To remove confusion as to two dates, we hereby declare that the date

of birth “August 18, 1941” and “August 18, 1942” refer to one and the
same person whose registered name is __________________;

6. We are executing this affidavit to attest to the truthfulness of the

foregoing statements and to establish the real identity of
_______________ and for all legal intents and purposes it may serve.
