For planning hours requested for payment, we must see the work (effort) completed for each hour requested. You are given a set number of hours based
on your program genre. If you have gone over the set number of hours, we need to see the proof.

You are paid .5 hours daily for daily prep (i.e., getting supplies ready for the day) when programs are in session – this time is paid as part of your
instructional time.

Once this plan is submitted, if any additional supplies are needed, you must submit your request on the “additional supply request” form: Click here to
submit additional supply request! Please note supplies are usually a 1–2-week turnaround, so please plan accordingly.

Note: Lesson plans must include all supplies needed. Please make sure your activity plan
describes how supplies will be used.

Description
Lesson Name: Fairy Portrait
Activity Plan:
Students will take portrait photos
through fairy lights. Depending on the
weather, the photos will be taken
inside or outside. It depends on
weather Forecast. Mason Jars, fake
flowers, and the provided mirror will
be used as props.
Students will take turns depending on
the number of supplies.
Students will pick one or 2 of their
favorite photos from this shoot.
A consent form will be signed by

Lesson Name: Ice flower
(weather permitting is best)
Activity Plan:
Students will take up-close photos of
items frozen in ice. This activity will
take place outside, but can be inside
too
Ice will be used to freeze the items
such as coins, feathers, leaves, and
other small items.
Adults will be the only ones to use the
hair dryer, it will be used to slowly
melt the ice.
Ice trays will be used to make the
subjects of the photos too.

Activity Plan:
Students will go through magazines
and cut out what items they want and
will arrange their chosen item on
standard paper. Students will glue
them down and create whatever
collage they want.
