SUBJECT: Reckless Driving resulting to damage of property.
DETAILS (use another sheet if necessary)
Facts of the Incident:

Please be informed that on or about 1351H today Thursday March 9, 2023; Assigned guard at pedestrian SG Eduardo Calles Jr.,
reported that an incident transpired along entrance of main gate pedestrian area involving One Zafra trucking, bearing plate #CBN-
9071, driven by Mr. Erwin Lo with helper Mr. Ricky Allado that upon entry at gate the driver tried to move backward due to traffic
congestion at entrance gate to give way to outgoing vehicle however driver failed to estimate the distance of gate and hit lower part
of the fence resulted to scratches and damage of the newly repaint gate.

Assigned guard took pictures as proof of evidence and immediately call the attention of duty officer who responded to the area to
verify the veracity of the report.

Evidence/s:

➢ Attached copies of photos of the incident including the damaged fence and the involve driver.
Findings:

➢ Finding shows that the incident transpired at main gate entrance involving driver Mr. Erwin Lo
of One Zafra Logistics Inc.
➢ That damage incurred to the main gate scratches and damage post.
➢ Driver was invited to security office for documentation purposes, however he opted to process first his deliveries before
attending the inquiry.
➢ That after his transaction driver submitted his voluntary written explanation quoted as saying “Ako si Erwin C. Lo driver ng
One Zafra trucking CBU-9071 pinaatras ako ng isang guard kanina inabot ng bahagya yung gate ngunit wala naming
damage.”
➢ Finding shows that his claim found no merit since the main gate suffered damage as reported.
➢ With this, we coordinate with One Zafra management thru Ms. Maricel Fernando since the driver failed to cooperate, the
customer service department thru Ms. Maricel Fernando promise to send personnel to estimate the repair of the damage
gate.

Conclusions:
• Wherefore, in view of foregoing we conclude that the driver Mr. Erwin C. Lo has committed a violation of reckless driving
or reckless imprudence resulting to damage of property.
• That the driver will be responsible for the immediate repair of the damage property.

• Risk Impact:
1. It may affect the image of the company.
2. It may emulate by others.
3. It may result to company loss.

Recommendations:

❖ Security highly recommends that the damage properly will be endorse to procurement department for the cost estimate
and repair of the damaged property.
❖ Also recommend to imposed disciplinary actions to violator to avoid repetition of same offense.
❖ Also recommend re-orientation to all inorganic employees the authority to operate MHE.

➢ Gate inspector immediately informed duty officer regarding the incident.
➢ Initiate investigation to determine the veracity of the report.
➢ Inform VSAZ Account Officer Cherol Mahinay regarding the incident.
➢ Send initial report to VIBER group for information purposes.

Corrective Action Necessary:

• To undergo reorientation for security & safety awareness.
