III. Problem Statement

a.) Presenting Problem:
A client may present with sinusitis and anxiety disorder symptoms may become the
focus of treatment or may represent a different, they been experiencing month, they expected to
gain promissory for her bill so that they will discharge to the hospital and financial assessment
for her treatment in the last month. The client try to solve the problems but they don’t have
enough money to treat the disease. He needs a financial help to sustain her medicine so that they
fully recovered, from her illness.

b.) Underlying problem:

Quinine studied at RNTVS. He is in Grade 7 and he needs to go to his school
because his father is canceling his studies because of his illness. And because of his
situation, he will stop his studies. she needs money for his sinusitis medicine and hospital
fees. That's why there are problems in his family, they don't have money to borrow and
he needs to take meditation for his anxiety because his mother is house wife and his
father is unemployed and his siblings are still studying. So they only rely on their small
business and their piggery . And they earn 6.000 per month and after harvesting for 3
months from their piggiry. that's why they earn a lot, almost 16,000. There are some
people who ask their relatives for money, but it's not enough for their hospital expenses
and for the maintenance of his medicine, so he needs financial assistance.

a.) The Client

Quinie pearl is a student from RNTVS high school at sto Domingo and she was
grade 7, she have one brother and two sister and she was younger to their siblings.
His father is unemployed. Because he is sick for almost two years. His mother take
care of their children. She can’t go to school of illness

Quinine's family have to spend a lot of money because his father is sick after Quinine is still in
the hospital and they will not be able to get any money. Their family monthly income is 6,000,
which is not enough to cover the hospital bill and maintenance of medicine.

General Objectives:
The client need to get, exercise daily to help feel good, maintain each eat well-balanced meals,
take a time-out, cognitive behavioral therapy it because is the most effective form of
psychotherapy for anxiety disorder.

VII. Recommendation

In regard to the above findings and information that were gathered, the worker highly
recommends that the treatment plan be thoroughly implemented and the client avail of possible
assistance from the agency or to be referred to another agency to be able to alleviate the
