Subject: Request for patient’s financial support regarding dental treatment

Respected sir,

I am Dr Kinza Hanif Bajwa PGR Prosthodontics working under the supervision of Prof. Dr.
Hina Zafar Raja. My patient 65 years old female, Parveen Saleem Regd:no 23-1899 is a
known case of myoepithelial carcinoma of the palate. She was operated on in 2019 by
maxillofacial department in Institute of Dentistry, CMH Lahore. Her previous surgical
history form is attached with the application. In 2022, she was diagnosed with recurrent
myoepithelial carcinoma for which she underwent radiation therapy 5 months ago. According
to current CT scan her disease is now stable.

After her first surgical resection patient was given a provisional maxillary obturator and its
cost was covered by PWS. During radiation therapy, patient lost two maxillary anterior teeth
and her remaining teeth are in poor periodontal health. Therefore, her previous obturator
needs revisions followed by fabrication of a new maxillary prosthesis by converting the
remaining teeth into overdenture abutments to preserve the remaining bone. However, patient
belongs to poor socioeconomic background and is not able to afford such treatment.

I am requesting to kindly consider this as a teaching case as it will not only benefit other post-
graduate residents and undergraduate students managing such cases but also helps to improve
patient’s quality of life. I shall be grateful for your consideration.
