AFFIDAVIT OF INCIDENT

I, MA. ELLAINE DORDAS ZHANG, of legal age, Filipino citizen,

married with postal and residence address at No. 12 B 23 L 13,
Anthurium St., Parkplace Village, Anabu 1-D, Imus City, Cavite,
Philippines, after having been duly sworn to in accordance with law
hereby depose and state:

1. That, I am the authorized driver and registered owner of the

following described-motor vehicle, to wit:

Make: TOYOTA
Denomination: SPORTS UTILITY VEHICLE
Plate: NFE1082
Body Type: SUV

2. That, on September 12, 2022, at around 5:20 A.M., the

abovementioned motor vehicle was parked un-attended at
abovementioned residence, when the rear right side portion of
the abovesaid motor vehicle was inadvertently and accidentally
side swiped by rear right side portion of a motor vehicle
described as TOYOTA Fortuner 4X4 Wagon, with plate
number ZAN189, which was reversing from their garage.

3. That, the aforementioned incident incurred an un-estimated

cost of damages for both motor vehicles.

4. That, I am executing this affidavit to attest to the truth of the

foregoing facts and for whatever legal purpose this may serve.
