DEMAND LETTER

We write on behalf of our client, MR. JULIUS S. BAUTISTA to

demand for the payment of NINE HUNDRED NINETY-NINE
THOUSAND PESOS [Php 999,000.00] reprsenting the amount due
from the contract of service entered into between JULIUS
BAUTISTA, PEDRITO BAUTISTA and DENNIS BAUTISTA represented
by his wife, ANNABEL BAUTISTA.

The amount due was incurred when Mr. JULIUS BAUTISTA took
care of your sickly father, PEDRITO BAUTISTA from the period of
January 2020 until the present. The amount due is supported by
a notarized contract of agrrement entered into on April 27, 2023
between you and your father PEDRITO BAUTISTA. A copy of this
contract of agreement is herein marked as ANNEX A.

We have already made several verbal demands for the

payment of this obligation. Please consider this letter as our LAST
AND FORMAL DEMAND for the payment of Php 999,000.00. You
are given five (5) days to pay the aforementioned amount.

indicated below or coordinate with our client.

Please be informed that we have given our assistance to our

client for the completion of FORM 01-_SCC Statement of Claim
pursuant to A.M. No. 08-8-7-SC which shall be ready for filing in
the event of your continued non-payment of the said demand.
In the event that the same is filed, we shall include in our
demand, the payment for damages and legal interests. We
advise you to please settle the amount on or before the
deadline to avoid serious inconvenience.
