Discharge date: 28 April 2018

Diagnosis : Right below knee amputation (BKA)

Hypertension
Peripheral vascular disease
Osteoarthritis
Age-related dementia
Moderate cognitive impairment

Social background : Retired postal worker


Little social interaction outside family for 6

Needs assistance with medication and
activities of daily living (ADL’s)

On admission : Infected right diabetic foot wound (diabetic


neuropathy).
Gangrene, pus, abscess.
Fever, chills.
Gram-positive cocci- Positive.

Medical progress : Given IV antibiotics


Vascular surgeon recommended BKA

Surgery performed without complication.


Changed to oral antibiotics and opiates for
pain.

Afebrile – wound is clean, dry, intact.


Requires assistance for ADLs

Nursing management: Change dressings daily.


Monitor surgical site for infection/drainage.
Check for fever/chills + other signs of infection.
Encourage oral fluids, nutrition.
Assist with ADLs and mobility.
Avoid decubitus ulcers.

Assessment : Good progress

Discharge plan : Discharge to Skilled Nursing Facility for acute


care and - physiotherapy.

nurse at the skilled care facility, Broomgrove Nursing and Convalescent Home, 30
Broomgrove Road, Sheffield S10 2LR.

In your answer:

● Expand the relevant notes into complete sentences


● Do not use note form
● Use letter format

The body of the letter should be approximately 180–200 words.
