WAIVER

any claim against the school for any untoward incidents that may arise while preparing and attending
for the scheduled Arts festival 2024, preparation for the said event will start on February 12 to 23 from
7:00am onwards, while the actual event will be on February 26 to March 1, 2024 from 7:00 am
onwards At Rizal High School Pasig City. The said activity aims to help the students to showcase
their talent/s in every art form, provide knowledge and experience most especially for grade 11 Arts
and Design students for they will the one who will organize the same event the following. I am fully
aware that I reported to school voluntarily intended for that purpose without pressures and availing it
free.

GUARDIAN’S / PARENT’S CONSENT

grant permission for his/her voluntary reporting to school for the scheduled preparation for RHS Arts
Festival on February 12-23 from 7:am onwards and attending the Actual Arts Festival on February 26
to March 1 from 7:am onwards. Further, I hereby agree to the terms and conditions with full
understanding to all provisions stated herein and waive my /our rights concerning the liabilities
described above.
