BOLEH DIGUNA PAKAI UNTUK BAYARAN CADANGAN BARU INSURAN HAYAT

ACCIDENTAL DEATH COVER

PERLINDUNGAN KEMATIAN AKIBAT KEMALANGAN

When we (Prudential Assurance Malaysia Berhad) receive a proposal form together with a deposit payment
equivalent to the first premium, and after we have issued this receipt to acknowledge such payment, we will provide
accidental death cover to the proposed life to be assured equivalent to the proposed Sum Assured (but not more than
RM500,000.00 for each life to be assured). This coverage shall be for a maximum period of 90 days from the date of
this receipt and shall only be given if the proposal is acceptable by us on standard rates.
Apabila kami (Prudential Assurance Malaysia Berhad) menerima borang cadangan bersama dengan satu bayaran
deposit setara dengan premium pertama dan setelah kami mengeluarkan resit ini untuk mengakui bayaran tersebut,
kami akan memberikan perlindungan kematian akibat kemalangan kepada hayat yang akan diinsuranskan yang
dicadangkan setara dengan Jumlah Diinsuranskan yang dicadangkan (tetapi tidak melebihi RM500,000.00 untuk
setiap hayat yang diinsuranskan). Perlindungan ini adalah untuk tempoh maksimum 90 hari dari tarikh resit ini dan
hanya akan diberikan jika cadangan boleh diterima oleh kami pada kadar standard.

If payment is made by cheque, the cheque must be honoured before this accidental death coverage is given.
Sekiranya bayaran dibuat melalui cek, cek tersebut mestilah diluluskan sebelum perlindungan kematian akibat
kemalangan ini diberikan.

This accidental death coverage is temporary and will stop once we accept or decline the said proposal, or once we
refund the first premium payment, or once 90 days from the date of this receipt has passed (whichever happens first).
The first premium payment will not be refunded if we have paid a benefit for this coverage.
Perlindungan kematian akibat kemalangan ini adalah sementara dan akan tamat sebaik sahaja kami menerima atau
menolak cadangan tersebut, atau sebaik sahaja kami memulangkan bayaran premium pertama, atau sebaik sahaja
tamat tempoh 90 hari dari tarikh resit ini (yang mana berlaku terlebih dahulu).
Bayaran premium pertama tersebut tidak akan dipulangkan sekiranya kami telah membayar sesuatu manfaat untuk
perlindungan ini.
