Lily has the ability to see the beauty in every moment, turning ordinary days into extraordinary celebrations with the guidance of her
parents. She loves to spread joy to everyone around her yet she needs to build her communication skills. She has enthusiasm for
learning though her teachers need to implement inclusive activities for her to cope up in class.

2nd quarter October 2013 Observation and testing Lily's progress was gradual but steady. Her speech improved

also improved, with Lily forming genuine connections with her
classmates. The occupational therapy was successful in enhancing
her fine motor skills, as observed in her improved hand-eye
coordination. Lily's academic achievements were improving. Her love
for art and music became focal points in her IEP, providing her with
avenues for self-expression and creativity that complemented her
academic journey.
3rd Quarter February Observation and testing Lily was be able to actively participate in every activity in class. She
assessment 2014 always eager of enjoyment for the new learnings. Lily can
socialize with her classmates. She always participates her therapy
and she develops her speech. Lily was be able to express herself
without a communication barrier.
FUNCTIONAL PERFORMANCE
Behavior Observation Lily was a very enthusiasm in her learning. She like to socialize
with her classmates bring a joy and positivity to them. She
actively participates in class. She gradually progress, show her
determination in build and enhance her communication skills
each passing day.
Fine Motor and Observation Lily demonstrates a strong ability to engage in activities that
Gross Motor Skills enhance her fine motor skills. She can hold a paintbrush properly
and create beautiful artwork, though she may need some
assistance in setting up her materials. Lily also shows a keen
interest in music, demonstrating her ability to strum a ukulele and
tap rhythmically on a drum. Her gross motor skills are evident in
her capability to participate in school physical activities. She can
walk, run, and even dance during music class, spreading joy with
her energetic movements.
Social Emotional Observation Lily has an exceptional ability to spread joy and positivity around
her, often seen with her infectious laughter and warm hugs. She
can correctly state her name, age, address, school, and teacher's
name. While Lily thrives in social interactions, she tends to form
deeper connections with a select group of classmates who she
feels most comfortable with. She is often seen sharing her
artwork or engaging in a duet during music class with these peers.
Despite her strong social skills, she may need encouragement to
broaden her social circle and interact more with other classmates.
