Specimen Nature: RENAL BIOPSIES

Specimen Site: KIDNEY
History: Recurrent hematuria.

Gross: SPECIMEN # 1 (KIDNEY, RENAL BIOPSY):

Specimen container is labeled with the patient's name and medical record number and is unmarked for
site.
Received in formalin is a single tissue core measures 0.6 cm. The entire specimen is submitted in a single
block.

SPECIMEN # 2 (KIDNEY, RENAL BIOPSY):

Specimen container is labeled with the patient's name and medical record number and is unmarked for
site.
Received in normal saline is a single tissue core measures 0.6 cm. The entire specimen is submitted in
Mitchel's solution for immunofluorescence.

Micro: Renal Biopsy Reporting Checklist

Glomeruli: (Evaluated on Hematoxalin & Eosin, JMS, PAS stains)

Number of Glomeruli: 30
Glomerular Change: The biopsy is composed of single core of renal parenchyma comprising cortex
only. No globally sclerotic glomerulus is seen. There is no increased mesangial cellularity or mesangial
matrix expansion. Three glomeruli show adhesions of peripheral capillary loop to Bowman's capsule
without podocyte hyperplasia. The peripheral capillary loops are predominant patent and no neutrophilic
infiltrate, endocapillary proliferation of crescent formation is seen. The glomerular basement membranes
are unremarkable. No spikes/holes are identified.

INTERSTITIUM (EVALUATED ON H&E AND TRICHROME):

Inflammation: Minimal chronic inflammation is seen in interstitium.
Fibrosis: No

TUBULES:
No tubular atrophy is seen.

VESSELS:
Unremarkable

IMMUNOFLUORESCENCE:

Diagnosis: KIDNEY, RENAL BIOPSY:

Features consistent with focal segmental glomerulosclerosis, tip variant.
No proliferative glomerulonephritis is seen.
Note: Immunofluorescence is negative and morphological features are predominantly unremarkable except
focal segmental sclerosis in few glomeruli. Features of IgA nephropathy or lupus nephritis are not seen
in submitted biopsy. Moreover focal segmental glomerulosclerosis and Alport syndrome can cause
hematuria. Clinical work up is suggested.
