NURSING PROCESS

Planning (Plan of Care): Establishes an individualized plan of care in collaboration with the patient, family, and the
health care delivery team, which is consistent with the assessment.
Implementation: Implements interventions to meet the specific needs of the patient, families, communities and
groups.
Evaluation: Evaluates the effectiveness of nursing care interventions; revises, and reprioritizes the plan of care.

Student comments: I have very good time management as I am able to complete assessments on time
and deliver medications in an appropriate amount of time. I understand how to prioritize patients
based on the severity of their needs. I communicate effectively, document procedures well, and ask
for help when needed. I don't mind asking questions and asking other more experienced nurses for
help. My clinical judgment is developing and I can see the improvement from past years. Instructor
comments: Rachel has consistently grown in skill during this semester. She consistently seeks and is
open to feedback, and produces excellent writing/documentation.
24. SUMMATIVE COMMENTS

NOTE: After completion of this evaluation, the student and clinical instructor will meet to review both the self-
evaluation and the CI's evaluation of the student. This will become part of the student's record and may be accessed
by the student and nursing faculty.
Student comments: I need to continue to grow in my clinical judgment and trust my instincts. I must
remember to speak up if I notice something wrong or abnormal. I need to gain more confidence when
speaking to providers. Most things I need to improve only will come with time and I understand it
takes time to develop the skills. Instructor comments: I agree with Rachel's comments above. She
tends to second-guess herself and is a bit apprehensive at times. Rachel can improve in her ability to
tackle difficult situations by trusting her instincts and continuing to expand her understanding of
various patho topics.
