UNDERTAKING TO PAY

(Re: Remittance of my PagIbig Salary Loan)

I, (Full Name)_________________________ have effectively resigned from the

Lizardbear Tasking, Inc. (hereinafter, the “Company”) effective (Date)_________________.
In this connection, per my records with the PagIbig as of to date, I have an outstanding loan
balance in the amount of Php. ___________________.

I undertake to personally remit the payment of said loan balance to the PagIbig and/or
enter into a payment schedule with the latter until my loan is fully liquidated. To this end, I have
requested the Company to cease any deduction of the said amount from my final pay,
equivalent to my accrued salaries and benefits.

In view of the foregoing, I am hereby releasing the Company of any obligations to remit
my loan payments to the PagIbig as of the effective date of my separation. Further, I release,
remise and forever discharge the Company, its respective successors-in-interest, stockholders,
officers directors, agents and employees, from any action, sum of money, damages, claims and
demands whatsoever arising wholly, partially or directly from the collection and remittance of
payment for the said PagIbig loan.
