AFFIDAVIT/CERTIFICATION

corporation duly organized and existing under the laws of the Republic of China with
office address at ________________________________, Philippines after having been
duly sworn in accordance with law, do hereby depose and state that:

1. I am the duly authorized representative of _________________________ which is

engaged in the business of steel manufacturing and is currently undertaking
construction of the steel plant in ____________________________________;

2. ___________ will be utilizing equipment and materials from China with Chinese
language instructions using Chinese Specialized High Technology for the
construction of the plant;

3. In relation to the foregoing, _________ is in need of SAFETY MANAGER;

4. _______ has exerted all efforts to headhunt a Filipino who is qualified and
trained for the position. ______ has, in fact, published the Job Vacancy with
Brigada Publishing on its (Insert Date of Newspaper issue) issue;

Copy of the Affidavit of Publication of _______- Publishing is hereto attached to

form an integral part hereof.

5. However, there were no applicants for the position thus, there is NO FILIPINO
AVAILABLE WHO IS QUALIFIED AND TRAINED to perform the tasks and
duties of SAFETY MANAGER.

6. I execute the foregoing statements to attest to the veracity of the same, and in
support for the fulfillment of the requirements with the Department of Labor and
Employment;
