A. The BSPC shall be kept in the General Service Office and shall be maintained for each building or structure.
B. This card shall be accomplished as follows:
1. LGU – name of the local government unit
2. Building/Structure ID No. – ID No. assigned to the building/structure 3. Name of Building/Structure – name of the building/structure 4. Description – description of the building/structure: i. Type of the building/structure - description as to the made of the building/structure (wood, concrete) ii. Location- the exact location such as the street, barangay where the building/structure is located iii. Floor area – total floor area of the building/structure iv. No. of floors – the total number of floors/storeys
5. Components - the components of the building/structure
i. Building/Structure – the core building/structure component ii. Air conditioning systems – the air conditioning units installed in the building/structure iii. Elevators/Escalators – elevators/lift installed in the building/structure iv. Others (Specified separately) –such as sprinklers, generators installed in the building/structure 6. Estimated Useful Life – estimated period over which the components of the building will be available for use. 7. Date – date of the source document 8. Reference – reference documents used as the bases in recording the transactions such as JV 9. Description – summary explanation of the transaction 10. Cost – amount of the transaction 11. Adjustment/s – the amount of adjustment/s made through JV 12. Carrying Amount – the difference between the total cost of the asset less the accumulated depreciation, accumulated impairment losses, and adjustments, if any 13. Maintenance History – Nature of Repair/Amount – the nature of repair (specify nature of minor or major repair and the cost of repair)
C. Transactions shall be posted from the source documents.
