(D)DESCRIPTION OF INCIDENT

On 16th February at around 1330hrs, Rahaman Lutfur was instructed by his supervisor Velayutham Dhayanithi to carried out gas cutting
(E)
works at Section J on a scaffold platform. However, his supervisor Velayutham Dhayanithi and Safety Coordinator Sundarraj C
Sugangtharaj did not carry out their duties by checking and ensuring that Rahaman Lutfur was provided with a safe working platform.
Rahaman Lutfur, being without any supervision, carried on his gas cutting works on a scaffold platform that was in the process of
dismantling.

CSEC Safety Personnel observed the unsafe act and issued a Near Miss Notification.

On 16th Feb 2024, at around 1330hrs I was instructed by my site supervisor Velayutham Dhayanithi to carry out gas cutting works using
scaffold platform at Section J. When I reached my work area, I saw that the scaffold platform was in already dismantled. Both my site
supervisor and my Safety Coordinator was not present to advise me on how to carry out my work activities. I then proceeded to carry out
my Gas Cutting Works despite the scaffold in the process of dismantling.

(G) ANALYZING THE INTERVIEW STATEMENTS

Based on the statement given by Rahaman Lutfur, the following can be derived:
- Site Supervisor Velayutham Dhayanithi failed to assess the work area of his worker before giving instructions.
- Site Supervisor Velayutham Dhayanithi failed to ensure check and ensure that a proper work platform is provided.
- Safety Coordinator Sundarraj Sugangtharaj failed to carry out his duty of inspecting the work area .

(L) RECOMMENDATIONS TO PREVENT RECURRENCE

- Safety Timeout shall be carried out.
- Both the Safety Coordinator and Site Supervisor was reprimanded.
- Site Supervisor and Safety Coordinator shall take a photo of the scaffold platform twice a day with date and time. One
in the morning and One in the Afternoon after Lunch Time and post it inside Zhao Yang WhatsApp Group.
