SUBJECT : Investigation report on Shooting Incident resulted to the Death of
John V. Conway

II MATTERS TO BE INVESTIGATED
3. To determine the facts and circumstances surrounding the alleged
administrative offenses committed by the subject at about 11:00 o’clock in the
morning September 09, 2022;
4. To determine if there are sufficient grounds to believe and hold the subject
PNP personnel administratively liable of the offenses charged and to further
determine the extent of their liabilities incurred;

III FACTS OF THE CASE

On or about 11:00 in the morning of September 09, 2022 Jeany H.
Conway reported the incident and seeks police assistance on the alleged
shooting incident transpired Purok NIA, Buayan, M’lang, Cotabato. John V.
Conway 30 years old, married, laborer, and a resident of Purok NIA, Buayan,
M’lang, Cotabato. He was allegedly shot by the unknown suspect on his
house on or about 11:00 in the morning.

Upon confrontation at M’lang Municipal Police Station Office, Leonida

Moreno Velado, a 48 years old, resident of the same place, and neighbor of John
 V. conway, based to her statement the suspect was dressed in a red jacket,
pants with sleeves, was tall, and was riding a black XRM and with no license
plate number. According to the eyewitness, she saw this around John V. Conway
house around 10:30 AM she thought he was one of the collectors who always
came to the said victim but later on she identified the person as Antonio Gregorio
Managuit resident of the same place. It was found that he did not pay attention,
and at that time she entered her house to rest, and around 11:00 AM she heard
banging and gun shot near her house. According to the eyewitness, she noticed
Antonio Gregorio Managuit racing out in John V. Conway house.
The investigation conducted by Police Inspector Nicko Garino Bases in
the alleged shooting incident revealed the witness and addressed her statement
regarding to the said incident.
Follow-up investigation conducted by the duty Police Inspector Nicko
Garino Bases in the place of incident particularly in the house of the victim where
the incident was happened, Purok NIA, Buayan, M’lang, Cotabato. In crime
scene they found out the three empty shell of Caliber 45, and the wallet that
determined the specification of the involved suspect.
In the statement of Jeany H. Conway, 30 years old, married, teacher and
a wife of John V. Conway residing in Puok NIA, Buayan, M’lang, Cotabato that
on September 09, 2022 at 9:00 in the morning he received a message from
unknown someone indicating threat message about to his husband John V.
Conway, Base on the statement of Jeany H. Conway before that day on
September 07, 2022 his husband had an controversy argument by someone
named Antonie Gregorio Managuit regarding to the properties of farmland.

IV INVESTIGATION/FINDINGS
In instance case under investigation in terms of those evidences and
witness that was recovered in the said place of incident and Antonio Gregorio
Managuit was of the person of interest.
In the follow-up investigation the conducted by the duty officer at the place
of the incident disclosed that the said evidences and the witness proved the
allegation that Antonio Gregorio Managuit pushed to be the suspect.

V CONCLUSION
After a careful perusal surrounding this particular incident, a fact of
comparison related to the shooting incident to the victim John V. Conway
matched the claim of the eyewitness and the evidences recovered
determined the primary suspect of killing the victim.

VI RECOMMENDATION/DISPOSITION
Wherefore, in view of foregoing, the undersigned respectfully recommends
that Antonio Gregorio Managuit will claim his rights and have an equal due
process of law. The said family of the victim will stand for the justice they seek
to address and the trial will be fair of.
