BUSINESS NAME REGISTRATION

I, ELIJAH REMBRANDT R. PANTALEON, Filipino, of legal age, and a

resident of No. 544 Martinez Street, San Juan, Morong, Rizal, under oath, do
hereby depose and state, that:

I am the registered owner of the business under the name and style of
PROMOTIONS PARADISE ONLINE SHOP with business address at San Juan,
Morong, Rizal;

The said business did not operate from May 2021 up to the present. I
hereby attest that the said business did not incur debt from any creditor during its
non-operation;

I am filing of the cancellation of business name registration of the

aforesaid business and that the execution of this affidavit is one of the
documentary requirements before they facilitate my application;

This AFFIDAVIT is being executed to attest to the veracity of the foregoing

statements in order to establish my intention to cancel the business name
registration of the aforesaid business, for purposes not contrary to law, morals,
good custom and public policy.

IN WITNESS WHEREOF, I have hereunto set my hand this 9 th day of
