PARENT’S AGREEMENT

We PARENT’S COMMITTEE/Guardian of TANDUH GUSU CHILD DEVELOPMENT

CENTER agreed to pay the ________________ PESOS (₱____.00) for our parent’s
participation fee and ___________PESOS (₱___.00) for monthly fee.

The ________________ (₱____.00) is intended for learning materials and other

needs for the Day Care Center, and ___________ PESOS (₱___.00) is additional honorarium
of the Child Development Worker.

The Parent’s/Guardian are voluntary contributing other expenses to the Day Care
Center and Day Care Children needed if necessary.
