1 TIRES:
V-4 RSS • No gouges, chunking, or flat spots.
22/30/40 • No bond failure.
V-4 • Adequate tread (if applicable).
V-2 • Remove all string, wrapping, and debris.
2 BATTERY:
V-5 • Fully charged/correct voltage and weight/no leakage

V-3 • Properly installed.


• Gates properly installed.

V-1 • Emergency disconnect functions properly.
• Check electrolyte level.
3 LIFT/LOWER, REACH/RETRACT, SIDESHIFT, AND TILT SYSTEMS:
V-5 V-3 • No damage to hoses or limit switches.
V-4 4 CONTROLS:
• Move smoothly without binding.
V-4 • Return to neutral when released.

5 GENERAL:
V-2 • Guards and covers installed and secure.

V-1 • Decals — (warning/safety/operational) in place and legible.


• Check floor under lift truck for any dripping oil that might indicate a leak.

3 DIRECTIONAL/SPEED CONTROL:
O-2 • Moves smoothly without binding.
• Returns to neutral when released.
• Controls speed and direction properly.
• Plugging functions properly.
O-1 O-4 • Speed limits function properly.
O-5 4 LIFT/LOWER, REACH/RETRACT, SIDESHIFT, AND TILT SYTEMS:
• Lift/lower button moves freely and returns to OFF when released.

O-7 RRS • All controls perform their functions as indicated.

O-4 5 EMERGENCY REVERSE BUTTON:


• Moves lift truck away from operator when pressed. Lift truck does not move again until directional/speed controls
O-2 move through neutral.
6 HOUR METER:
• Records hours of lift truck operation.
7 BRAKES:
• Handle moves freely without binding.
O-5 O-1 • Stops lift truck in specified distance (refer to the Maintenance Manual).
• Brake functions properly (travel circuits disabled).
O-3 O-4
8 SAFETY:
O-7 RCS • All limit switches function properly (refer to the Maintenance Manual).
• All warning and working lights are functional.
O-6 • Travel and lift functions shut down when battery is disconnected.

Do not operate this truck if an unsafe condition is discovered.


Lock/Tag the truck out of service and notify your supervisor immediately.
