APPROVED FOR AWARD TO THE LOWEST BIDDERS/SUPPLIERS WITH THE LOWEST QUOTATIONS AS TO
INDICATED SUBJECT TO EXISTING LAWS AND REGULATIONS.

WHEREAS, the BAC Members convened last July 22, 2023 at 8:30 in the morning at the HE Room.

WHEREAS, the purpose of the meeting was to open the Request for Quotation canvassed by the canvasser Mr.
Glendon L. Alon with advertisement No. 2023-7-002nlc-Elem dated July 24, 2023

WHEREAS, there were three stores involved: Procurement Service

WHEREAS, the table below indicates the price quotations for each items.

WHEREAS, Plaza Drygoods has the lowest price quotations on all items needed;

NOW, THEREFORE, upon a joint motion and by a unanimous accord of the members of Bids and Awards
Committee (BAC) present during the meeting;

RESOLVED, as it is hereby resolved, to award to Plaza Drygoods all the items needed;

RESOLVED FURTHER, that copies of the Purchase Order be prepared and submitted for the purchase of the said
items;

UNANIMOUSLY APPROVED
