PRE-ASSESMENT. PAR-Q (Physical Activity Readiness Questionnaire)
This questionnaire will tell you whether it is necessary for you to seek further advice from your
doctor or a qualified exercise professional before becoming more physically active.
Read the questions carefully and answer each one honestly.

 Has a Doctor ever advised you to avoid physical activity or only engage in √
certain exercises due to a heart condition?
 Do you experience chest pain during physical activity? √

 Have you experienced chest pain during non-physical activity within the √
past month?
 Have you ever lost your balance or consciousness due to dizziness? √

 Do you have any bone or joint issues that could worsen with physical √
activity?
 Are you currently taking medication prescribed by a Doctor for heart √
disease or high blood pressure?

 Do you know of any other reason why you should not do physical activity? √

Let's see what the questionnaire revealed about your fitness.


 If you answered YES to any of these questions, visit the University Clinic to see the
doctor and talk with him/her before you start engaging in physical activity. Tell the
Doctor about the PAR-Q and which questions you answered yes.
 If you answered NO to all the PAR-Q questions, you could take part in all physical
activity.
 Exceptions: Even if you answered “NO” to all questions, you should delay taking the
physical activity if you're temporarily ill with a cold or a fever, or other pertinent
conditions. Inform your P.E instructor for this matter.
