at Shualful Office, San Felipe Site

Dear Sir,

This is in reference to the above subject. We have thoroughly researched various internet
service providers and only Converge ICT Solutions Inc. provides internet connectivity within
our satellite office and the most suitable option for our internet needs.

To fast track our application, the easiest way is to apply internet connection through personal
application. It means it will be under the name of our employee but the company will shoulder
the payment. This is the most expeditious and economical application considering the tedious
process of corporate application as well as higher rate of fees.

Here are the details of our request:

Plan: Converge Internet Plan 1500 (200 Mbps)

Monthly Payment: Php 1500 per month (24 month lock-up period)
Payment Terms: 1-month advance payment of Php 1500 upon installation
Connection Fee: Php 2500 upon installation
